HOW DO I GET AN NHS COVID RECOVERY PASS?

Please be advised that the surgery does not provide recovery letters for travel.
Some private GP’s/private practices do offer this as a chargeable service, but this is not an NHS duty so please do not contact the surgery to request a certificate as we will be unable to assist you.

HOW DO I GET AN NHS COVID PASS TO PROVE MY VACCINATIONS?

To get a record of your COVID-19 vaccinations on the NHS App, if you are already registered on Online Services you will be able to download the NHS App and sign in by verifying your identity, you can then request an NHS COVID Pass on the App. If you are not yet registered for Online Services please email cwccg.cheylesmoresurgery@nhs.net with a copy of 1 form of Photo ID and we will send you the login details required for you to register and this will enable you to verify your identity on the NHS App. You can also view your COVID vaccinations on the Immunisations section on Online Services.

ONE OF MY VACCINATION DOSES IS NOT SHOWING ON MY RECORD

If one of your vaccination doses is not appearing on your record or the NHS App then please send us a copy of your vaccination card via email and we will be able to update this for you. If you do not have a vaccination card please provide us with as much information as you can referring to the place that you received your vaccine, brand name and batch number if possible. Please note that we may have to get into contact with the Hub Site that you received your vaccination from in order to get this updated and this can take some time.

WHEN WILL I BE INVITED FOR A COVID BOOSTER?

When you are eligible for a COVID booster vaccination you will be contacted via SMS or email. If we do not have a contact number for you please contact Reception to update this.
