We offer face to face, telephone and video consultations. To book one of these\u00a0appointments, please either:<\/p>\n\n\n\n<p class=\"has-nhs-yellow-background-color has-background\"><strong>One appointment is for one person.\u00a0If you want another member of your family to see the doctor or nurse, please book a further appointment.<\/strong><\/p>\n\n\n\n<ul><li>Call our Surgery Reception on:\u00a0024 7650 2255 between the hours of 8am\u00a0to 1pm\u00a0and 2pm to 6:30pm<\/li><li>In person<\/li><li><a href=\"https:\/\/www.patientaccess.com\/\">Online<\/a><\/li><li>When arriving at the surgery for your appointment, please use the check in screen\u200b<\/li><li>Please make one appointment for each member of the family who needs to be seen<\/li><li>We try to keep to time but please be patient if someone before you takes longer than planned<\/li><li>It is Practice Policy to allow patients to choose whichever Doctor they wish to attend in the Practice<\/li><li>If you would like a chaperone during your consultation please inform the doctor or receptionist<\/li><li>If you require a same day appointment with any of the GP&#8217;s\/GP Registrars please call the Surgery reception on 024 7650 2255 at 8am as these appointments are booked on a first come first served basis.<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Please help us<\/h2>\n\n\n\n<p>If you are not able to attend your appointment please let us know in time so that the time can be used for someone else. If you are late for an appointment you may be asked to re-book.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Missed Appointments and DNA\u2019s<\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">Do you feel frustrated when you cannot get an appointment?<\/h4>\n\n\n\n<p>Every year wasted appointments cost NHS \u00a3162 million. This can cause serious delays in treatment for other patients. On average approximately 150 patients each month Did Not Attend (DNA) their appointment. This means the patient did not turn up for the appointment and did not contact the surgery in advance to cancel or change the appointment. This currently results in approximately 40 hours of wasted clinical time each month.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link\" href=\"https:\/\/www.cheylesmoresurgery.nhs.uk\/clinics-and-services\/appointments-tests-referrals\/appointments-with-the-doctors\/cancel-an-appointment\/\">Cancel an appointment<\/a><\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion\">\n<div class=\"wp-block-coblocks-accordion-item\"><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Additional information <\/h2>\n\n\n\n<div class=\"wp-block-coblocks-accordion\">\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title\">Can I bring someone to accompany me to the Appointment?<\/summary><div class=\"wp-block-coblocks-accordion-item__content\">\n<p>We are generally very happy for patients to bring with them a carer, relative or even a friend. This often helps, particularly when they know you well and it allows them to tell us any observations they would make about you, which can help us in undertaking our assessment. Ultimately this will lead us more quickly to make an accurate diagnosis and therefore help you more.<\/p>\n\n\n\n<p>Also, having someone with you means they can prompt you to ask questions that you may have forgotten, and after the appointment they can help in reminding you what was discussed.<\/p>\n\n\n\n<p>Occasionally we might ask for them to leave but this would be unusual.<\/p>\n\n\n\n<p>If you require&nbsp;interpretation services please contact us in advance of you appointment and we will arrange this.<\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title\">If you have a suspected infectious disease<\/summary><div class=\"wp-block-coblocks-accordion-item__content\">\n<p>Please inform reception if you suspect an infectious disease, as this will enable us to deal with it appropriately during your visit to protect you, other patients and staff.&nbsp;<\/p>\n\n\n\n<p><\/p>\n<\/div><\/details><\/div>\n\n\n\n<div class=\"wp-block-coblocks-accordion-item\"><details><summary class=\"wp-block-coblocks-accordion-item__title\">Giving Consent for Treatment<\/summary><div class=\"wp-block-coblocks-accordion-item__content\">\n<p>You have the right to accept or refuse treatment that is offered to you, and not to be given any physical examination or treatment unless you have given valid consent. If you do not have the capacity to do so, consent must be obtained from a person legally able to act on your behalf, or the treatment must be in your best interests.<\/p>\n\n\n\n<p>Your valid consent (agreement to the course of action) is needed for the treatment that\u2019s offered to you before any physical examinations or treatment can be given. If you haven\u2019t given your consent, you can accept or refuse treatment that\u2019s offered to you.<\/p>\n\n\n\n<p>It\u2019s important to be involved in decisions about your treatment and to be given information to help you choose the right treatment. You can make an appointment with any of the&nbsp;Doctors at the practice. As medical records are now computerised,&nbsp;all Doctors,&nbsp;at any site can access your details. If you prefer, you can make an appointment with a&nbsp;Doctor of your choice.
